Machine Incident Record
The "Incident Record" page is used to provide the details of a machine incident, including the cause & correction, time lost, associated cost, and priority level.
Incident Header
Assigned To
User, department, or work center responsible for the incident report.
Incident Status
Current status of the incident record, reflecting whether the incident itself has been resolved. Any user with access to an open incident record can close it by clicking the Close button, or cancel it by clicking the Void button. An incident record can only be reopened by the user who closed it, or by a Setup Administrator.
Status Information
Describes the last change to the record's status, as well as the User ID of the user responsible for that change.
Comments
Information provided when the record's status was last changed.
Incident Record
Machine Name
Name of the machine for which the incident occurred.
Occurrence Date & Time
Date/time the incident occurred.
Maintenance Number
Internal designation for the machine maintenance check that revealed the issue, if applicable.
Preventative Maintenance Checks
Type of machine maintenance check that revealed the issue, if applicable.
Cause
The cause of the recorded incident.
Corrective Action
The correction undertaken in response to the incident.
Time Lost
Amount of machining time lost due to the incident, in minutes.
Description
Detailed description of the incident that occurred and the response.
Priority Level
Importance of the machine incident and its resolution.
Corrective Action Comments
Information concerning the corrective action taken. This can be a justification of the corrective action selected, a description of its effects or ramifications, etc.
Requested Completion Date
Date by which the incident report should be completed.
Estimated Completion Date
Date when the incident report will be completed.
Incident Cost
Labor Cost
Additional personnel cost for repairing the machine, in dollars (or local currency).
Material Cost
Cost of materials needed to repair the machine, in dollars (or local currency).
Repair Cost
Cost of requisitioning repair for the machine, in dollars (or local currency).
Other Cost
Miscellaneous costs associated with machine incident, in dollars (or local currency).
Total Cost
Total cost of repairing the machine, in dollars (or local currency). Automatically calculated from other costs.
