FAIR Form 2
This form should be used if any of the part’s materials, special processes, or functional testing are defined as design characteristics.
Materials and Processes are required AS9102 fields.
Inspections are Net-Inspect provided optional fields.
Functional Tests and Acceptance Reports are Conditionally Required (CR) AS9102 fields.
1. Part Number (R)
Number of the part. Automatically imported from Form 1.
2. Part Name (R)
Name of the part as shown on the drawing. Automatically imported from Form 1.
3. Serial Number (CR)
Serial number of the part. Automatically imported from Form 1.
4. FAIR Identifier (R)
Reference number that identifies the FAIR. Automatically assigned by Net-Inspect.
5. Material or Process Name (CR)
Name of applicable materials or special processes.
Each line item may be entered in the “Material”, “Process”, or “Inspection” sections of Form 2. (“Inspection”, a non-AS9102 defined section, is suitable for specifications that rely only upon a physical or visual inspection of the part.)
6. Specification Number (CR)
Spec number of the material/process/inspection entered in Field 5. If the line item is a COTS item or standard hardware, then provide its reference number.
7. Code (O)
Any required code from the customer for material or process listing, as applicable.
8. Supplier (CR)
Name, address, and/or supplier code of the organization performing the special process or providing the material. Name and address are mandatory if a code is not sufficient to properly identify the company.
9. Customer Approval Verification (CR)
Used to indicate whether the process or material is approved by the customer. Select “Yes” if approved, “No” if source is not approved but approval is required, and “NA” if approval is not required.
10. Certificate of Conformance Number (CR)
Certificate number (for example, special process completion certification, raw material test report number, standard catalog hardware compliance report number, or traceability number).

After entering a document description in this field, a document related to the corresponding line item can be uploaded by clicking the line and then clicking its "Upload/Remove Documents" button. Clicking the document name will open it in a separate tab. A description of the attachment(s) can be entered in the adjoining field.
11. Functional Test Procedure Number (CR)
Functional Test Procedure number identified as a design characteristic.
12. Acceptance Report Number (CR)
Functional test certification indicating that test requirements have been met.
